Q1: What insurance plans does the Forest Hills Health Center participate with?

We accept all the major HMOs, private and government insurances. Call us with all your insurance participation questions at 718.997.7100 or 718.896.4100.
For insurance plans that require referrals, they must be obtained from your Primary Care Physician and presented to our office on the day of your appointment.

Q4: How to get to Forest Hills Health Center?
A4: By subway: The E, F, R, M trains at the 71st Continental Avenue stop. Make a right onto Austin Street and walk heading towards Yellowstone Blvd.

By Bus: the Q60 and get off by MacDonald Park near Yellowstone Blvd, walk up to Austin Street and turn left; The Q23 bus along Austin Street stops near the Eddie Bauer Store.
